Field notes The care

Leave room for a real conversation.

Is a health-history form the whole visit?

The form records your health history and medicines. It starts the visit; it does not answer all your questions.

Myth to check

Completing a fixed form is the same as being assessed.

Rethink that tirzepatide assumption

A form can start the review. Care needs enough information for the person treating you to make a decision, with room for questions.

A person must review the form

The guidance checked on 26 September 2026 says an assessment cannot rely only on a fixed form that leaves medical questions unresolved.

A form reviewed by a doctor or other licensed health professional who can ask more questions is a different process.

The medical boards’ guidance also requires a chance for you to raise further questions about your care.

Make the health history easy to review

Have the names of medicines you take ready, along with questions you want answered.

Mention any cancer in you or your family and any serious stomach problems. Bring records of past inflammation inside the belly, gallstones, or surgery for those problems. The person treating you can check those records against the label’s warnings.

The cancer warning names a rare cancer called medullary thyroid cancer. The records help check the exact illness, even if you do not know its medical name.

Pregnancy plans, birth-control pills, other diabetes drugs, eye disease from diabetes, and upcoming surgery also belong in that review.

Tests depend on the assessment

The general federal prescribing rules checked on 26 September 2026 did not set a blood-test requirement for every prescription.

That does not decide whether tests are needed for your care; state rules and proper medical practice still matter.

When an assessment calls for an exam or testing, the boards’ guidance calls for arranging it or referring the patient.

Ask how the decision will be explained

Ask what the person treating you is trying to treat, why that product is being considered, and what happens if it is not suitable.

Before paying for ongoing care, ask how follow-up works and what charges continue if no prescription is issued.

A menu is not an assessment

The boards’ guidance says a patient picking a treatment from a menu cannot, by itself, justify prescribing.
